Athletic Clearance Process

If you or your student plans to participate on an El Monte athletic team, all required athletic clearance steps must be completed electronically through HomeCampus prior to participation in any tryouts, practices, or games. Athletic clearance is required every school year and for every sport. This ensures that all student-athletes are medically eligible and properly documented before joining a team. The athletic clearance steps include:

  1. Student Details
  2. Parent/Guardian Details
  3. Medical History
  4. Additional Questions
  5. Signatures
  6. Files (red = required)
    1. Current, Valid Pre-Participation Physical Exam (PPE) / Sports Physical
You may print the El Monte Athletics Clearance Packet yourself, or pick it up in either the Activities office or the Sports Medicine Clinic (Room 50). This packet includes the following forms:

      • Physical Examination Form (page 1)
This form must be completed by your student’s licensed medical provider during the Pre-Participation Physical Exam (PPE).
      • History Form (pages 2 & 3)
This form must be completed by the student and parent/guardian before the medical examination. It provides important background health information for the provider.
      • Medical Eligibility Form (page 4)
        • This form must be completed and signed by a licensed healthcare provider:
          • Physician (MD or DO)
          • Nurse Practitioner (NP)
          • Physician Assistant (PA)
          • Important: Forms signed by providers such as chiropractors (DC), registered nurses (RN), or other non-authorized medical personnel cannot be accepted.
      • PPEs/Sports Physicals are valid for one (1) year from the date of the exam.
        • We strongly encourage student-athletes to complete their PPE/Sports Physical in June or July whenever possible. Completing your physical during the summer helps ensure clearance is valid for the entire school year and reduces interruptions to athletic participation.
      • PPEs can be completed with your primary care physician, urgent care, or during campus sessions during scheduled times throughout the year.
    2. Proof of Insurance:
      • All student-athletes are required to have current and valid health insurance to participate in El Monte athletics.
      • If a student does not currently have health insurance, optional coverage plans are available through Myers-Stevens & Toohey. Families may review plan options and enroll online.
